Return of Medication

Good evening,
If your child has a medical plan and medication on file at the school (e.g. epi-pen, Advil, puffers, etc...), they will be coming home to you on Thursday. Please make any necessary changes on the medical form you receive. Upon returning the medical plan in September, on the first day of school, please ensure there is a physician's signature accompanying it; this is now the Simcoe Muskoka's Catholic District S.B. policy.
If you know in advance that your child will not be here on the last day, please send us a note indicating their last day and we will ensure the medication and form goes home with them.
